feat: add expert-mini/expert-cqrs-mini pieces and fix permission fallback

- Add expert-mini and expert-cqrs-mini pieces (ja/en)
- Add new pieces to Mini and Expert categories
- Fall back to readonly when permission mode is unresolved instead of throwing
